Overview
Telemonitoring at home with Avistacare
Telemonitoring, sometimes called remote patient monitoring, allows certain health readings to be collected at home and shared with your care team between in-person visits. Using simple monitoring devices, clients can record measurements such as blood pressure, heart rate, weight, blood oxygen or blood sugar as part of their daily routine.
Those readings give our team a clearer, more continuous picture of how you are doing. Instead of relying only on what can be observed during a scheduled visit, we can follow trends over time and notice changes early. When a reading falls outside the range set in your plan of care, our team can follow up, adjust visit schedules, or communicate with your physician as appropriate.
Telemonitoring is designed to work alongside skilled nursing and telehealth, not to replace them. The specific measurements, devices and monitoring schedule are determined by each client's needs and plan of care, and our staff will help you and your family learn how to use the equipment with confidence.
What our Telemonitoring service includes
Depending on your plan of care, telemonitoring may involve:
Vital Sign Tracking
Regular readings such as blood pressure, heart rate and blood oxygen recorded at home.
Weight & Blood Sugar Monitoring
Tracking measurements that matter for conditions like heart failure or diabetes.
Trend Review
Our team reviews readings over time to understand how you are doing between visits.
Timely Follow-Up
When readings change, our team can check in, schedule a visit or contact your physician.
Device Setup & Training
Help setting up equipment and learning how to take accurate readings.
Family Peace of Mind
Added reassurance that changes in health are less likely to go unnoticed.
Who can benefit from Telemonitoring?
Telemonitoring can be especially helpful for people managing chronic conditions such as high blood pressure, heart disease, diabetes or lung conditions, as well as clients recently discharged from the hospital who need closer observation during recovery.
It also supports independence. Many clients appreciate being able to live on their own terms at home, knowing that their care team has a regular view of how they are doing.
